He was predeceased by: his parents, Rob Middleton and Amy Thomasson. He is survived by: his children, Meredith Middleton, Ashley Hagaman and Jason Middleton (Crystal); his stepmother Gail Middleton; and his siblings, Nancy Tucker, Mike Middleton (Spot) and Greg Middleton. He is also survived by nieces, nephews, and a host of friends.

Memorial contributions may be made to Hawkins Cemetery and mailed to Mrs. Sharon Bradford, 12288 Highway 136, Rising Fawn, GA 30738.
