She was predeceased by: her parents, Roy Hawkins and Dollie Hawkins Sr.; her former husband Barry Crutchfield Sr.; and her brothers, Curtis Hawkins Jr. and Teddy Hawkins.
She is survived by: her children, Jack Crutchfield (Donna), John Mark Crutchfield and Donna Gailer (David); her sisters, Dorothy Richardson (Louis) and Diane Morgan (Larry); her sister-in-law Judy Crosby (Clinton); and her grandchildren, Tripp Crutchfield (Amy), JD Crutchfield (Kacey), Alaina Crutchfield, Shannon Purcell (Chris), Hannah Gailer (Lauren) and Thomas (Cody).
