He was predeceased by: his parents, Thomas Lewis and Willie Mae Lewis. He is survived by: his wife Shirley Lewis; and his children, Chuck Lewis of Texas, Doug Amick (Betty), Joey Amick (Tiffany) of Georgia, Kenny Amick (Karen) of South Carolina, Cindy Broome (Dale) of Georgia and Nicole Lewis of New York. He is also survived by grandchildren and great grandchildren.
