He was predeceased by: his parents, Johnny O'Neal and Maxine Evans; and his siblings, Jimmy Evans and Janet Evans. He is survived by: his daughter Sarah Hoffman (Jason); and his cousins, Mary Wells, Houston Evans and Jeanette Mullins.
