He is survived by: his wife Candice Michelle Chambers Cook; his children, Levi Cook, Aundrea Cook of Temple and Lee Cook of Roswell; his mother Mable Cook of Temple; his siblings, Diane Woods of Douglasville and Frank; his sister-in-law Connie; and his brothers, Clay, Tommy Berry, Cliff Sanders, David Smallwood, Olin Wiggins and Ronnie Robinson. He is also survived by nieces and nephews.
