He was predeceased by: his parents, Ralph Clack and Edna Grace Welbron Clack; and his former wife Sharon Jane Priest Clack. He is survived by: his wife Cindy Clack; his children, Phil Clack, Jr. (Lacy) and Lucy Clack; and his sisters, Francis Hansard and Carol Fosbrook.
