He was predeceased by: his parents, Hayes Jack, Sr. and Josephine Gideon Mack; his wife Donna Marie Allen Mack; and his brothers, Bobby Ray Mack, Sr., Hayes Mack, Jr. and Billy Spratton.
He is survived by: his children, Jonathan Mack (Rashanda) of Covington, David Mack (Kendra) of Snellville, Richard Wikstrom of Toccoa, Christopher Wikstrom of North Carolina and Tina Margaret Smith of New York; his grandchildren, Jonathan Trey Mack, Shayla Mack and Aneesa Mack; and his siblings, Larry Mack of Royston, Jerry Mack of Toccoa, Georgia, Betty Rainey of Royston, Goldie Marks of Toccoa, Marjorie Boyce of Commerce, Linda Mack of Toccoa, Jacqueline Manley of Martin and Louise Dewberry of Pittsburg, Pennsylvania. He is also survived by seven grandchildren.
