He was predeceased by: his grandparents, Pauline and Edward J. Smyke Sr.; his uncle Kenneth Smyke; his grandparents, Verna M. Berthelsen and Robert C. Berthelsen, Sr.; and his stepgrandmother Sara Berthelsen.
He is survived by: his parents, Lisa Smyke Berthelsen and Chris Berthelsen; his brothers, CJ Berthelsen (Merricks) and Ryan Berthelsen; and his half-sister Stephanie Merrifield (Mathew). He is also survived by numerous aunts, uncles, cousins, and a niece and nephew.
