He was predeceased by: his parent Arlin Monroe Forysth; his wife Hazel Irene White Forsyth; and his brother Larry Dean Forsyth. He is survived by: his grandchildren, David Wright, Doug Wright, Daniel Wright, Zach Thompson, Morgan Mazurek and Emily Gann. He is also survived by three daughters.
In lieu of flowers, the family requests that memorial contributions be made in memory of Mr. Forsyth to the Fairview Baptist Church Building Fund, P. O. Box 442, Rockmart, GA 30153.
