He was predeceased by: his father Charlie Jay Polk. He is survived by: his children, Haley Polk of Texas and Kaiden of Fayetteville, GA; his mother Janice Polk of Riverdale, GA; and his brothers, Stephen Polk and Michael Polk. He is also survived by numerous aunts, nephews, nieces, and cousins; and many friends.
