He is survived by: his wife Renee Butler; his children, Jeannie Thomas (Steve) and James Sheffield (Darlene); his grandchildren, Courtney Parker (Wesley), Casey Smart (Jonathon), Britini Blackmore (Jonathan), Rebecca Thomas and Alex Thomas; his great grandchildren, Tormund Blackmore and Boone Smart; his siblings, Tim Formby, Harold Formby, Jr. and Angela Formby Butler; and his pet Mack. He is also survived by nieces and nephews.
