He was predeceased by: his parents, Richard Forth and Edith Forth. He is survived by: his wife Micheline Marie Forth; his children, Erika Forth (Kevin Miller) and Derek Forth (Sara Shefchik); his granddaughters, Edie, Audrey and Norah; his siblings, Gerry Forth and Joan Mann; and his brother-in-law Claude DesRoches. He is also survived by several nieces and nephews.
