He was predeceased by: his parents, J. W. Middlebrooks and Lilly Margaret Raper Middlebrooks; and his siblings, Doris Beavers, Willie Middlebrooks, Jimmie Middlebrooks and Barbara Davis.
He is survived by: his wife Elaine Peavy of Kathleen; his daughters, Crystal (Dale Thorn), Danielle (Jarrod Clark) and Michelle (Josh Grube); his grandchildren, Chasen Thorn, Madelaine Thorn, Connor Clark, Emerson Clark, Keller Peaster, Tate Peaster, Dani Reese Grube and Hayes Grube; and his siblings, Lillian Gresham of Byromville, Gail Singleton of Madison and Larry Middlebrooks (Rebecca) of Montezuma. He is also survived by many nieces, nephews, great-nieces, and great-nephews.
