He is survived by: his father Homer Fred Phagan, Jr. (Linda); his children, Derek Chase Phagan, Chandler Blake Phagan and Tristan Chance Phagan; his sister Natalie Phagan O'Brien; and his close friend Carrie Anne Martin. He is also survived by numerous nieces, nephews and cousins.
