He was predeceased by: his parents, Benjamin T. Cook, Sr. and Bessie Overstreet Cook; and his brothers, Chester Cook and Albert Cook. He is survived by: his children, Benjamin T. Cook, III of Ochlocknee, Rebecca Cook of Valdosta, Jonathan Cook (Jill) of Statesboro, Elizabeth Cook of Thomasville and Alicia Cook of Thomasville; his grandsons, Nathan Cook and Ben Samples; and his siblings, Carolyn White of Dixie, Jim Cook of Griffin and Terry Cook of Dixie.
