He is survived by: his wife Betty Lois Smithwick of Nashville; and his children, Jack B. "Chuck" Smithwick, Jr. of Nashville and Martha Humphries of Waycross. He is also survived by two grandsons; four granddaughters; 12 great grandchildren; three brothers-in-law; one sister-in-law; and several nieces and nephews.
