He was predeceased by: his father James Monroe Cook; his daughter Felicia Cook; and his sister Sheila Cook. He is survived by: his wife Angie Cook; his children, Wendy Cook, Jody Cook and Melinda "Meme" Cook; his siblings-in-law, Maria and Donald Smith; his grandchildren, Angelia Hughes (Nathan), Takeshia Smith (Russell), Lee McCorkel (Hannah), Cassandra Long, Jonathan Cook (Kayla), Travis Long (Maria), Austin Arp, Jessi Davis, Savannah Cook and Toby Davis; and his great grandchildren, Eli Hughes, Kiersten McCorkel, Kinley McCorkel, William McCorkel, Caroline Hughes, Joseph McCorkel, Freya Long, Julia Smith, Skyler Cook, Asmund Long and Serenity Cook. He is also survived by numerous nieces, nephews.
