He was predeceased by: his father Johnny Mitchell Cook; and his sister Felicia Leah Cook. He is survived by: his children, Jonathan Cook (Kayla) and Savanah Cook; his granddaughters, Skyler Cook and Serenity Cook; his mother Angie Howard Cook; and his sisters, Wendy Cross and Melinda Cook. He is also survived by nieces, nephews.
