He is survived by: his parents, Jeptha Polk and Shirley Blair Polk; his wife Kathy Polk; his children, Brittany, John Thomas, Briana and Tristan Greene; his grandchildren, Finley Thomas and Harper Thomas; and his sister Tracy Dycus. He is also survived by nieces, nephews.
