Smith (Joe Cwik) of Gainesville; her stepdaughter Pam Davis of Forsyth; her sister Kay Willoughby (Jim) of Milledgeville; her grandchild Cameron Davis; her nephew Jon Willoughby (Ginger) of Statesboro; and her great-nephew Tyler.

In lieu of flowers memorial contributions may be made to Alzheimer's Association.
