He was predeceased by: his parents, Clayton Polk and Ella Polk. He is survived by: his wife Mary W. Polk; his children, Craig Polk (Angela), Kristy P. Woodard (Randy) and Tracy Polk; his niece Sherry Sloan (Ben); his grandchildren, Holly Bradford (Richard), Kacy Polk, Brooke Henson (Jack), Karleigh Andrieux (Ryan), Micah C. Woodard, Bart Mullis (Diana), Tara Gonga and Savannah Averos (Remy); and his great grandchildren, Elodie, Lillian, Leela, River, Willow and Londyn.
Visitation was held on Thursday, December 12th 2024 from 1:00 PM to 2:00 PM at the Fairhaven Funeral Home (4989 Mt Pleasant Church Rd, Macon, GA 31216). A funeral service was held on Thursday, December 12th 2024 at 2:00 PM at the Chapel of Fairhaven Funeral Home (4989 Mt Pleasant Church Rd, Macon, GA 31216).
