He was predeceased by: his parents, Thomas Cook and Annie Cook; and his wife Jenelle Cook. He is survived by: his children, Troy Cook (Barbara), Perry Cook (Sandy), Mitch Cook and Sheila Cook; and his siblings, Glenn Cook (Connie), Dennis Cook (Susan), Denise Hooper and Kathy Taylor (Randy). He is also survived by numerous grandchildren, great-grandchildren and numerous nieces, nephews and cousins.
