He was predeceased by: his parents, Boyd Lewis and Louise Carter Lewis; and his brother Tony Lewis. He is survived by: his wife Faye Miller Lewis; his children, Chris Lewis (Christy), Hollie Lewis Baird (James "Bill" Smith), Craig Lewis (Kristen) and Chase Lewis; his stepdaughters, Vanessa Derrett (Harley) and Crystal Prather (Nate); and his brothers, Freddie Lewis and Ronnie Lewis. He is also survived by seven grandchildren.
