She was predeceased by: her parents, John Slamkowski and Julia Slamkowski; her husband Herbert; and her brothers, John and Thomas Slamkowski.
She is survived by: her children, Daniel Hoffman (Lori) of Kennesaw, GA, Julie Baranowski (Michael) of Dyer, IN, Janet Hoffman of Lafayette, IN and David Hoffman of Palm Harbor, FL; her brother Edward Slamkowski; her sisters-in-law, Kathleen Slamkowski and Terry Slamkowski; and her grandchildren, Amy, Andrew, Lynn and Rachel. She is also survived by three great-grandchildren; and many nieces and nephews.
