He is survived by: his children, Tammy Reed (Kevin) of Milledgeville, Crystal Polk of Jesup and Jamey Polk of Jesup; and his siblings, Diane Johnson, Debbie Thompson, Angelia Walker of Baxley, Larry Polk of Dublin and Tommy Polk of Milledgeville. He is also survived by several grandchildren and great grandchildren.
