He was predeceased by: his parents, Ernest Harry Smith and Nellie Ruth Smith. He is survived by: his daughters, Jessica Smith Burgamy (Eric) of Tennille, Kimberly Smith Withrow (Blake) of Harrison and Megan Smith of Sandersville; his grandchildren, Kayla Burgamy, Bailey Burgamy, Blake Withrow II, Raelyn Smith, Ashlyn Smith and Nickolas Burgamy; his siblings, Carol Blue (Charles) of Wrightsville, Linda Denton of Mitchell, Steve Rentz (Peggy) of Louisville and Tommy Smith (Tammy) of Indiana; and his close friends, Curtis and Teresa Blount.
The family suggest donations be made to the Pulmonary Fibrosis Foundation.
