He is survived by: his mother Carol Maddox; his children, Amber Maddox and Hunter Shumate; his grandchildren, AJ, Zoe and Le'Veon; and his sisters, Leslie Doden (Mitch), Melanie Polk (Tommy) and Lori Collins (Tim). He is also survived by many nieces and nephews.
