He was predeceased by: his father Wallace Bouchard. He is survived by: his daughter Madison Leigh Bouchard of Oakwood; his mother Anne White Bouchard of Gainesville; his sister Allison Bouchard Conley; his brother-in-law Chris Conley; and his nephew Cale Conley of Gainesville.
In lieu of flowers, the family has requested donations be made to Juvenile Diabetes Research Foundation.
