He was predeceased by: his parents, Herbert Addy and Susie Wilson Addy; his wife Faye Davis Addy; and his children, Cynthia Michelle Addy and Herbert Preston Addy.
He is survived by: his daughter Susie Addy Grizzle (Darrell); his grandchildren, Tiffany Smith (Isreal) and Wesley Grizzle; and his great grandchildren, Jaxon, Nora-Faye and Crimsen Jack Smith. He is also survived by his family dearly, and adored his two grandchildren and his three great-grandchildren.
