He was predeceased by: his parents, Paris Ballard and Millie Ballard; and his brothers, Kenneth Ballard and Mark Ballard. He is survived by: his wife Betty Ballard; his daughters, Kim West (Phillip) and Janice O'Neal (Mike); and his sister Kathryn Gilley. He is also survived by four grandchildren and twelve great grandchildren.
