He was predeceased by: his parents, Willie McIntosh and Odessa McIntosh (Jones) of Elberton, Georgia.; his wife Willie Beatrice McIntosh; his daughter Maxine Eberhart (McIntosh); and his siblings, Jake of Knoxville, TN, Lenwood, Cofer, Harry McIntosh of Cleveland, OH, Roena Booker of Elberton, GA, Rita Hurst, Mary Lewis of Gainesville, GA, Carrie Middlebrook and Willie Sue Samuel.
He is survived by: his children, Cathy McInthosh Hickman (Randy Hickman) of Athens, Christopher McIntosh (Iris McIntosh), Houston, Otis, McIntosh Jr. (Tabitha Stokes McIntosh) of Athens, Willie McIntosh of Pueblo and Andrew; his son-in-law John Eberhart of Hull; his grandchildren, Shaquita and Amber Eberhart; and his great grandchildren, Ziya, Tarail Parks, Hikari, Minori Walton, Matthias and Rucker. He is also survived by nieces, nephews, cousins, and friends.
