He is survived by: his wife Janice Jordan Forsythe; his children, Joey Forsythe (Carman), Priscilla Forsythe and Jeffrey Forsythe; his stepson Kenny Davis (Karen); and his sisters, Linda (James Sikes), Sue (John Eustace) and Mitzi Forsythe. He is also survived by several grandchildren, great grandchildren, nieces, nephews, other family and friends.
