He is survived by: his wife Peggy Middlebrooks; his children, Amy Middlebrooks Tapley (Chris) of Columbus, GA, Abby Middlebrooks of Greenville, SC and James Daniel Middlebrooks (Jenny) of Callahan, FL; and his grandchildren, Matthew Middlebrooks, Beckett Tapley, Morgan Middlebrooks, Brooks Young and Avery Young.

A memorial service was held on Sunday, January 12th 2025 at 3:00 PM at the Bill Head Funeral Homes and Crematory - Duluth Chapel (3088 Duluth Hwy, Duluth, GA 30096).
