He was predeceased by: his parents, Casper Dee and Juanita Long Cook; and his brothers, Robert Cook and Richard Cook. He is survived by: his wife Barbara M. Cook of Doerun; his sons, Carl Cook of Moultrie, Michael Cook of Valdosta and Andy Causey (Wendy) of Doerun; and his grandson Phillip Causey of Doerun.
