He is survived by: his wife Phyllis Booth; his daughters, Stephanie (Terry Belue), Lesley and Christina Booth of Hebron, IN; and his sister Carolyn Pinkston of Indianapolis, IN. He is also survived by six grandchildren and 3 great grandchildren.
