She was predeceased by: her husband Fred George Azar, Jr; her parents, Joseph Slade and Mary Taylor; and her brother Joe W. Slade. She is survived by: her children, Lori Sise, Mark (Jackie Azar), Louis (Judy Azar) and Doug (Karen Azar); and her grandchildren, Chastity, Nichole, Chris, Chad, Lorin, Kelly, Timothy, Brianna, Ryan and Cody. She is also survived by 14 great-grandchildren; several nieces, nephews and cousins.
