He is survived by: his daughters, Carolyn Cook (Terry Kaminsky), Rebecca Cook Conley and Dolores Cook Ellison (Ken Cox); his grandchildren, Cindy Conley Hogan (Tony Hogan), Richard, Monte and Hollis Chadwick Ellison; and his great grandchildren, Cheyenne Hogan Lange (Dan Lange), Katerina Conley and Katherine Ellison.
