He was predeceased by: his parents, J. C. Cook and AnnMarie Cook; and his brother Donald Cook. He is survived by: his wife Mary Gilbert Cook of Conley, GA; his children, Bonnie Cook (Wes Allen) of Dallas, TX and Scott Cook (Carol Cook) of Forsyth, GA; his grandchildren, Maggie Cook-Allen and Ryan Molock (Katie); his great grandchildren, Jaxon Molock and Taylor Molock; and his sisters-in-law, Jeanne, Charlotte, Betty, Fay and Sue (Gerald). He is also survived by nieces and nephews.
