He was predeceased by: his parents, John Elmer O'Brien Jr. and Claire Margaret Cifaldi O'Brien. He is survived by: his wife Shirley O'Brien; his daughter Monica; his son-in-law Matthew Wurtz; his sons, Michael Fernandez and John O'Brien IV; his sister Maureen Melfa; his grandchildren, William Wurtz, Sean Wurtz, Kaitlyn Fernandez and Colin Fernandez; his aunt Dru Cifaldi; his nieces, Cindy Melfa and Sandy Forsyth; his great-nephew Anthony Forsyth; his great-niece Ashlynn Forsyth; and his father-in-law Fred Haas.
