He was predeceased by: his parents, J. P. Smith and Betty Beam of Washington. He is survived by: his wife Mary Cook Smith; his daughter Donna; his son-in-law Jonathan Mulkey; his brothers, Mark A. Smith and Joel E. Smith; his grandchildren, Katherine and Madalyn Mulkey; his nieces, Hayley Phillips and Kimberly Crawford; and his nephew Chris Smith.
In lieu of flowers, memorial contributions may be made to Peeples Funeral Home to cover his final expenses.
