He is survived by: his children, Doug "Dougie" Cook, Tommy Cook and Runda Tate; his grandchildren, Coty Cook (Katie), Shelby Cook, Matthew Cook, Kiersten Cook, Jessica Mantooth (Chris) and Benjamin "B. J."; his great grandchildren, Hunter and Alyssa Mantooth; and his sister Cathy Stout.
