He was predeceased by: his parents, Grady Bradley Cook and Dorothy Mozell Fortner Cook. He is survived by: his sisters, Davita Starr (Jimmy), Barbara Meyer (Bob), Kathy McKenzie (Johnny) and Deborah Watson (Ron). He is also survived by numerous nieces and nephews.
