He was predeceased by: his stepfather Delbert L. Cook; and his sister Robin D. Cook. He is survived by: his wife Shelley Collins (Scott); his children, Kyle D. Collins (Ana), Haley A. Collins (Sean) and Logan G. Collins; his grandchildren, Ryleigh and Emerson; his parents, Joe E. Collins Sr. (Cathy) and Shirley L. Cook; and his siblings, Joe E. Collins Jr. (Debbie), Michael "Chad" Collins (Octavia) and Shellie Miller (Wayne). He is also survived by cousins, nieces, and nephews.
In lieu of flowers, the family requests donations be made to the Brain Aneurysm Foundation.
