He is survived by: his wife Dianne Renee Cook; his children, Jason Richard Cook, Jr. (Ellen), Justin Ryan Cook (Elise) and Katelyn Danyelle Cook (Kevin); his grandchildren, Kaylee, Madilynn, Hunter, Jaycob, Riley, Ava, Jasper and Memphis; and his siblings, Joseph Cook (Paige) and Maria Thornton (Bryan).
