He was predeceased by: his grandparents, Jesse Lee Conner, Eris Haley Conner and Claudine Moon Hawk; and his father-in-law Sammy Lee Slaton. He is survived by: his parents, Sherry Conner Hawk and Thomas Clifford Hawk, Jr. (Tommy); his wife Amanda Slaton Hawk; his children, Mackenzie Jewel Hawk and Eric Thomas Hawk; his sister Tangie; his brother-in-law Ron Merritt; his grandfather Thomas Clifford Hawk, Sr.; his mother-in-law Elisia Slaton; his siblings-in-law, Michelle and Jimmy Lazenby; and his nieces and nephews, Kenneth Hawk, Abigail Merritt, Erica, Sam Bowden, Haley Lazenby and Madison Lazenby.
