He is survived by: his mother Rachel Alexander Middleton; his siblings, Terrie Smith (Stan) and Gene Middleton (Ashlee); his nephews, Payton Smith, Patrick Smith and Travis Smith (Samantha); and his great-nieces and great-nephews, Jaxson and Cason.
