He was predeceased by: his parents, Adam Formanek and Sophie Formanek (Kolycznski). He is survived by: his wife Carolyn Formanek (Soinski); his children, Cherie Formanek, Ronald Formanek, Robert H. Formanek and Cynthia Audirsch (Formanek); and his sister Dolores. He is also survived by twelve grandchildren, and two great grandchildren.

In lieu of flowers, donations may be sent to the Union County High School Fishing Team at 153 Panther Way, Blairsville, GA 30512.
