She was predeceased by: her parents, Donald Hart and Doris Hart; her husband Greg Jacobsen; and her brother Edward Hart. She is survived by: her children, Todd (Chasidy Oakes), Patrick (Lisa Oakes) and Jenifer Weyandt of Cleveland, OH.; her grandchildren, Sierra Mills, Alexis Oakes, Chance Schmeltz, Addi Schmeltz, Danielle Schmeltz, Angel Moss, Michael Moss, Alisa Moss, Garrett Oakes, Kyle Oakes, Taylor Weyandt and Ariel Silknitter; and her great grandchildren, Ivory, Charlotte, Jackson Mills and Jameson McLain.
