He was predeceased by: his wife Sandra L. Sullivan; and his parents, Donald E. Sullivan and Helen Sullivan. He is survived by: his children, Kenneth D. Sullivan of Auburn, Georgia and Margaret L. Smith of Cincinnati; and his grandchildren, Katelyn and Adelynn Smith.
