He was predeceased by: his father Earl Middlebrooks; and his brother Ruben Middlebrooks. He is survived by: his wife Alice Middlebrooks; his daughters, Amy (Rick Moore), Sandy (Stephen Mallory) and Amanda (Jerry Blackwell); his mother Nell Middlebrooks; his siblings, Marie Hart, Joe Middlebrooks, Ruth Sykes, Nora Allison, Jeff Middlebrooks, Jackie Middlebrooks and Jerry Middlebrooks; and his stepchildren, Belinda, Jeremiah Outwater and Steven Gabriel. He is also survived by 14 grandchildren, and many other family and friends.
